If you are in receipt of benefits or on a low income you may qualify for Housing Benefit. Claims can only be assessed and paid once Revenues and Benefits have received all the required information. Housing Benefit claim forms are available from Housing Service Centres or from the Revenues and Benefits section telephone: 0845 351 0020.

DO NOT DELAY - IT COULD RESULT IN THE LOSS OF HOUSING BENEFIT.

Important things to remember:

• Answer all the questions on the form, even if the answer is 'NONE'
• Supply National Insurance numbers for yourself and your partner
• Provide original documents of income and details of savings for claimant and partner and any non-dependent in the household
• Two forms of identification
