The Community Care Unit provides a housing service for vulnerable older people, people with physical disabilities, people with learning disabilities, people with mental ill health, young people leaving care and vulnerable 16 / 17 year olds who are in housing need.

We have a dedicated team of staff who advise individuals and agencies on housing and support options.

Referrals to Community Care may be made by statutory or voluntary agencies, potential users and carers which may include:

• Neighbourhood Offices
• Health services
• Social Services
• Self-referral
• Community Mental Health Teams
• GP's
• Education
• Carers
• Specialist Organisations, e.g. NSF, MIND
