The Community Care Housing Unit provides a specialist housing and advice service to individuals, statutory and voluntary agencies, where a housing need has been identified.

We work with vulnerable people who are diagnosed with mental ill health, learning disabilities, physical or sensory disabilities.  We also work with young people including young people leaving care and young families.

Our mission is to improve the quality of life for vulnerable people in Sandwell by providing a person centred approach to housing and resettlement. 

We provide advice and assistance on access to housing and support services to people who are:

• Being discharged from hospital, residential or rehabilitation units
• Moving on from supported housing projects or supported lodgings
• Young people Leaving Care
• Young families including teenage parents
• People with learning disabilities who need accommodation as part of the supported living model
• And people who need an adapted property.

Through a team of experienced staff including two dedicated Community Care Housing Officers we undertake:

• Joint assessments of housing need
• Gather information to support the allocation of appropriate accommodation and support
•  Advocate for the involvement of statutory agencies for an holistic approach to ensure vulnerable people maintain independent living
• Guide our customers along a variety of housing pathways from referral and assessment to housing and resettlement.
